The password change must stay below the domain's lockout threshold. The policy now carries lockout_threshold and lockout_window, from the domain or from the fine-grained policy that applies. AD-2's read allow-list names the new attributes.

Split load_config into per-section readers, use contextlib.suppress and a single condition where ruff asked, remove stale noqa comments, add the SIM and RUF100 rules, share the sign_in test helper, align AD-6 with the code and record the remaining deferred findings.

Authorization-code flow with PKCE through an idp adapter (with a fake), ID
token validation, identity and roles resolved from the directory with a short
cache, refusal of unknown, disabled and locked accounts with one generic page,
sessions that end when the account becomes unavailable, sign-out that ends the
server-side session, role decorators and the trusted-proxy client address rule.
Adds the OIDC_* and PUBLIC_BASE_URL configuration.

LDAPS directory adapter with certificate verification, allow-listed reads and
writes, escaped filters, password verification and change, password policy
and account state, plus an in-memory fake. Adds the LDAP_* configuration,
python-ldap with its image libraries, the CA file mount and an opt-in live
test for the test OU.
